Fellowship Square Phoenix offers three levels of assisted living services: supervisory care, personal care, and directed care. Before admission, residents complete a health assessment with our assisted living director to determine the level of care that best fits their needs. Once the appropriate level of care is determined, the director will review the care plan with the resident and their representative upon admission.

Personal Care
In personal care, residents receive assistance with daily care activities such as bathing, dressing, and grooming. Our staff also assists with ambulation, transferring, medication management, and administration.
Residents receiving personal care services must be able to eat independently, request assistance, and effectively communicate their care needs to the staff.
